A 52-year-old man presents to your district hospital at 2000h with SOB, abdominal pain and fever. He has had an elective endoscopy and colonoscopy that morning. He does not have any major past medical history except for GORD and FHx of bowel Ca. Vitals on arrival : T 38.1 HR 110 BP 110/70 Sats 95% RA GCS 15. A CT of his chest and abdomen is performed.

The patient deteriorates atier returning from CT scan to resuscitation area in ED. He complains of increasing difficulty in breathing. His neck seems swollen and has crepitations on examination. He becomes agitated and his sats drop to 90% on RA and his BP to 90/60.
