Case: SOB and chest pain

A 74-year-old man has presented via ambulance to your tertiary ED with shortness of breath and chest pain for the last 2 days. He has a history of type 2 diabetes mellitus and ischaemic heart disease, having had a stent in his right coronary artery 6 years ago.
